Erectile dysfunction (ED) is a common, age-related disorder that diminishes quality of life for affected men and their partners. While most erectile dysfunction cases are now recognized as organic in origin, both organic and psychogenic causes often coexist to reduce sexual function in men with erectile dysfunction.
   Identification of cardiovascular risk factors should be a routine part of the evaluation for ED and is as important as taking the patient's sexual, medication, and psychosocial histories. Involving the patient's partner in evaluation and management is often valuable. Treatment with phosphodiesterase type 5 inhibitors is effective in restoring sexual function for most men with ED, but patients and their partners should be encouraged to make an informed choice from among all available treatment options.

We evaluated the current evidence for the use of acupuncture to treat erectile dysfunction (ED). Of the four studies included, one randomized controlled trial (RCT) showed beneficial effects of acupuncture compared with sham acupuncture in terms of response rate, while another RCT found no effects of acupuncture. The remaining two studies were uncontrolled clinical trials. Collectively these data showed that RCTs of acupuncture for erectile dysfunction treatment are feasible but scarce. Most investigations had methodological flaws, e.g. inadequate study design, poor reporting of results, small sample size, and publication without appropriate peer review process. The evidence is insufficient to suggest that acupuncture is an effective intervention for erectile dysfunction treatment.

Pelvic floor exercises are effective as erectile dysfunction treatment

To examine the role of pelvic floor exercises as a way of restoring erectile function in men with erectile dysfunction. In all, 55 men aged > 20 years who had experienced erectile dysfunction for more than 6 months were treated with either pelvic floor muscle exercises (taught by a physiotherapist) with biofeedback and lifestyle changes (intervention group) or they were advised on lifestyle changes only (control group). Control patients who did not respond after 3 months were treated with the intervention. All men were given home exercises for a further 3 months. Outcomes were measured using the International Index of Erectile Function, anal pressure measurements and independent (blinded) assessments. After 3 months, the erectile dysfunction of men in the intervention group was significantly improved than in the control group. Control patients who were given the intervention also significantly improved 3 months later. After 6 months, blind assessment showed that 40% of men had regained normal erectile function, 35% improved but 24% failed to improve. This study suggests that pelvic floor exercises should be considered as a first-line approach for men seeking long-term treatment of their erectile dysfunction.

ED in teenagers
Dr. John P. Mulhall, a researcher at Memorial Sloan Kettering Cancer Center in New York City says young men with ED may experience psychological harm that interferes with their sexual function in adulthood, and the earlier treatment is started the more effective it will be. Dr. John P. Mulhall reports about 40 males ranging in age from 14 to 19 who came to one of three different specialized ED treatment centers for help. Half of the teens reported having a decreased or absent sex drive, 30 had difficulty achieving spontaneous erection during sexual encounters, and 35 had trouble maintaining erections. When a physical cause of ED is diagnosed, treatment is often possible, with hormone therapy if endocrine problems are identified, or vascular surgery for the few with blood vessel problems. For young men whose ED problems are related to a bad sexual experience or loss of confidence, treatment involved medication that will allow them to achieve erections when they have sexual relations along with psychological counseling. I have read a study of using acetyl-L-Carnitine coupled with Viagra as an erectile dysfunction treatment in diabetics. Has any study results revealed that this erectile dysfunction treatment is effective with those who suffer high blood pressure and are taking high blood pressure meds such as beta blockers, Lisinopril, Valsartan, and hydrochlorothiazide.
